It turned out to be (2) for us we looked at the app pools in iis and ecp was mapped to v4.0 for ms.net We switch to 2.0 and iis restarted and it appears to be working now. I access the mailbox by owa, when click on the option, i straight away logout from owa without any notification. I've check the exchange server and i see that on ecp the mailbox do not results on o365!

So i've check on portal.office.com and i saw that the user still has the mailbox on exchange online

Incredible things is that i can log into the mailbox from both environment!! Owa in exchange server and on outlook.office365.com!!! Today i generated a new certificate request on each of the 4 exchange servers and received new certificates from digicert, installed them on each of my exchange servers and selected these new certificates in the iis bindings for default web site and exchange back end I also took one of the certs that was generated for one of the exchange servers and added it to the owa service on my kemp load.
